The assessment collects your name, email address, and phone number if you give one. It also collects the state you live in and your answers. Our web host keeps standard server logs, which include IP addresses.

Why we collect it

We use it to reply to you and to prepare your file for clinical review. That is the only reason.

What we do not do

We do not sell your information. We do not trade or rent it. We do not use it for advertising.

Who sees it

The care team handling your request, and the independent licensed clinician who reviews your case if you go forward. Our web host stores the data because it runs the server.

How long we keep it

Enquiries are kept while they are active and for a reasonable period afterwards. Ask us to delete yours and we will.

Your choices

Email us to see what we hold, correct it, or have it deleted. Depending on where you live you may have further rights, and we will honour them.

Cookies

This site sets no tracking or advertising cookies. Your language and your light or dark preference are stored in your own browser and never leave it.

Nothing here is medical advice, a diagnosis, or a prescription. Reading it does not create a clinician-patient relationship.

The assessment is not a decision

Completing it does not enrol you, charge you, or qualify you for any treatment. A licensed clinician decides what is appropriate after reviewing your labs.

Who provides care

Clinical services are provided by independent licensed clinicians and licensed 503A compounding pharmacies, not by this website.

Compounded medications

Compounded medications are not FDA-approved. FDA has not evaluated them for safety or effectiveness.

Eligibility

Services are for adults 21 and over. Availability varies by state.

Prices

Prices are in US dollars and can change. The price shown to you before you are charged is the price that applies.

Emergencies

This site is not for emergencies. If you have a medical emergency, call 911.

Your telehealth consent and the HIPAA Notice of Privacy Practices come from the clinical provider who treats you. They are issued at intake. This website does not issue them. We are a marketing and enrolment site, not the covered entity. Ask your clinician for copies.
